1. You aren’t aware of where your hands are at the top of your backswing.
2. Your first move into transition does not match where your hands were at the top of your backswing.
3. Your sequence from the top to impact is not controlled in the correct order.

What about if your somewhere in the middle lol
Before I watch this video I just want to point this out…. I was struggling with this exact issue this week, I started using a flat’ish type swing and it felt so ugly I just couldn’t commit. I was hitting it so well but it felt so wrong.
